Nutrition Services
Nutrition Services include nutrition education & counseling. We provide medical nutrition therapy for the management of various conditions, including cardiovascular disease (hypertension, high cholesterol, congestive heart failure), obesity, underweight, reflux, irritable bowel syndrome, gastroparesis, dysphagia, Crohn's disease, Ulcerative Colitis, ostomy, diverticular conditions, Celiac disease, food allergies, cirrhosis, diabetes (T1DM, T2DM, gestational), chronic kidney disease, post kidney transplant, nutrition-related anemias, cancer, tube feeding, and other conditions that can lead to malnutrition or altered nutrient needs.
The nutrition counseling process uses evidence-based interventions and goal-setting that are tailored to optimize nutrient status and overall health.
A large majority of our appointments stem from a physician referral. However, a referral is not required to schedule. If a referral comes from an outside healthcare facility, it can be faxed to FHFP.
